The structure of this paper is an index of links to information, which, read in order, should be a more-or-less complete project. It is meant as more than just a history. It is meant as a history, an analysis, and a preventative measure. It is meant to get as many stories as possible "on the table" and to bring the voices of the people involved into the stories. It is not exhaustive, it is not perfect. It never will be.
